This advocacy guide has been co-produced with people who provide unpaid care and is designed to help carers to better understand what to expect and how to prepare for supporting the person they care for on a virtual ward (‘Hospital at Home’). It speaks to carers raising their everyday questions and prompts actions, thoughts and practical steps from carers. We hope it will simplify a lot of worries and concerns carers may have, empower carers to feel part of the team and to ask questions, and ensure that there is a two-way dialogue between virtual wards teams and unpaid carers.
